Send the signed ticket and a completed claim form (Spanish | Hmong) to the above address. Using certified or registered mail is suggested. Retain a personal copy of both sides of the ticket and claim form.

Where in Wisconsin is the Winner Wagon?
The Winner Wagon is periodically stationed around the state to provide players another option to claim winning tickets in addition to the Madison & Milwaukee Redemption Offices or mail.
All prizes will be paid by check since the Winner Wagon does not have cash to pay prizes. Only prizes under $200,000 may be claimed.

The Validations & Player Relations team will be on site at Festival Foods in Eau Claire (3007 Mall Dr) to assist with the redemption of winning lottery tickets. Prizes up to $200,000 may be redeemed at this event, with all payments issued by check.
